THE NATURAL AND PREVALENCE OF MEDICATION ERRORS IN A TERTIARY HOSPITAL IN INDONESIA

Objective: Medication error is the most common errors in a hospital setting and serious issue that intimidate safety of patient may cause mortality morbidity. The aim this study to explore rate medication reporting an Indonesian hospital.
 Methods: This retrospective descriptive. research was conducted at dr. Soeradji Tirtonegoro General Hospital Medical Center from January June 2020. reporting, which reported for six months, sample study.
 Results: On period, 105,171 prescribing sheets were collected. 9.5% total are categorized as Error (ME). highest incident ME (88.24%), then followed by transcribing (7.61%), dispensing (4.02%), last administration (0.13%).
 Conclusion: occurred stage because not supported electronic prescription. inevitable event during process. But, incidents can be minimized implementing some preventive strategies improve safe drug use.
